2009 NORTHWEST REGIONAL
INDOOR SOCCER CHAMPIONSHIP

TEAM ELIGIBILITY

The 2009 Indoor Soccer Northwest Regional is open to affiliated competitive teams from USYSF (for example IYSA),

Club, and US Futsal, as well as independent teams. The recommended maximum team roster size is 10 players for all teams. Officially, the maximum roster size is 10 players (U10 and below), 14 players (U11 and U12) and 12 players (U13 and above). Rosters from club teams registering with their club name are limited to 4 guest players (from the same club or from another club). Otherwise (i.e. for non club teams), there are no limits on guest players. Note that once a team has been checked in with a tournament official (before the team’s first game) their roster is set and no more players can be added to the roster. Double rostering is not allowed.

TEAM CHECK-IN INFORMATION

Location and Date/Time: 45 minutes before your first game at the Boise Indoor Soccer arena at 11448 W. President Drive.

Team Documentation: The following documentation will be required at check-in (for guest players see below).

(1) BIS Player Cards (Free). Each player needs their photo taken at the arena. The facility will then print a player card (FREE) before the first game. Proof of birth date is required to print the card not currently in the data base. Process takes about 60 seconds per player. Please arrive early before your first game to complete this process. Players currently playing at the arena will need to get a new player card (previous cards will not be available). These cards, together with an official tournament roster, will be used for player identification by referees at each game. BIS Cards will only be issued the first day of the championship (no staff will be available to do that in consequent days).

NOTE: BIS Liability Waiver. Entry Fees paid do not include insurance for injuries to players, coaches or managers. Parent or legal guardian must sign the BIS (Boise Indoor Soccer) Waiver for players (one form per player) before the BIS player card is issued. Please, get this form in link table below. Players currently playing at Boise Indoor Soccer (BIS) do not need to sign an additional waiver.

(2) Club Affiliation Documentation (Club Teams only):

Club teams will need USYSA affiliates Player Cards (for example IYSA cards), US Club Passes, OR Equivalent USSF State affiliation documentation (for example a USYSA team roster).

(3) ONE Official Tournament Rosters (please, download a copy in the link table below).

(4) Medical Releases. Each team should have their players medical releases on hand and each club is responsible for their own teams’ and players’ well-being. A medical release form is provided in the link table below for players without one.

Rosters from club teams registering with their club name are limited to 4 guest players (from the same club or from another club). Otherwise, there are no limits on guest players. All guest players must also get a free BIS Player Card (as the rest of the players) before their first game so therefore, their parents must have signed the BIS Liability Waiver Form and have their medical release forms and birth certificates as well.

Team coaches are recommended to obtain some sort of written agreement (for their own records) from the clubs from which they are using guest players.

INSURANCE

All IYSA (Idaho Youth Soccer Association) players will be covered by player insurance since this tournament is sanctioned by IYSA (in addition to being sanctioned by the USIndoor Association and sponsored by the National Indoor Soccer Council).
